Summary
Virginia DMAS is retiring the fax submission process (804-452-5456) for DMAS-80 nursing facility admission/discharge forms, DMAS-421A, and admission packages effective June 30, 2026. All submissions must transition to email (nfportalentries@dmas.virginia.gov) with secure encryption and required portal error screenshots. Providers must also ensure enrollment information is current in the PRSS provider portal to avoid claims payment disruptions.
Action Required
By June 30, 2026: (1) Billing and nursing facility admission staff must stop using fax number 804-452-5456 for DMAS-80, DMAS-421A, and state-owned nursing facility admission submissions. (2) Update all internal workflows, templates, and provider communications to direct submissions to nfportalentries@dmas.virginia.gov using secure email only. (3) Verify your practice has encryption services; if not, contact nfportalentries@dmas.virginia.gov to request a secure email option. (4) Train all staff submitting level of care entries to include screenshots of MES portal error messages with every inquiry. (5) Verify that all nursing facility and hospice provider locations have current enrollment, contact information, and license details in the PRSS provider portal (https://vamedicaid.dmas.virginia.gov) to prevent claims payment denials. Before June 30, 2026: Billing team must audit current processes using the fax number and document all affected workflows. Update billing system contact lists and protocols. Failure to transition to secure email and maintain current PRSS enrollment will result in lost submissions and claims payment disruptions for Fee-For-Service and MCO members.
