Summary

Starting August 1, 2026, new providers joining the network must submit a W-9 form with both legal name and DBA name (if applicable), plus official IRS documentation (SS-4 or 147C forms) as part of their onboarding application. Current in-network providers are not affected unless they have organizational changes.

Before August 1, 2026: Practice administrators must ensure any new providers being added to the practice have completed W-9 forms with both legal and DBA names plus official IRS documentation (SS-4 or 147C) ready for network application submission. Current providers only need to update if there are name or TIN changes.
