By September 30, 2026: Billing and prior authorization teams must implement the following: (1) Configure billing system to require prior authorization for all Siltuximab (Sylvant™) claims with distinct authorization pathways based on indication (Castleman's disease = 12 months; CAR T-cell CRS = 1 month; lymphocyte engager toxicities = 1 month; KICS = 12 months); (2) Update prior auth submission requirements to mandate documentation of HIV-1 and HHV-8 status for all requests; (3) Create clinical criteria validation rules in the authorization workflow to verify: for Castleman's disease—multicentric vs. unicentric designation, disease status (active idiopathic, relapsed/refractory, progressive, fulminant/severe, or unresectable), and HHV-8 negative status; for CRS—grade level and corticosteroid/anti-IL-6 refractoriness; (4) Configure continuation of therapy re-authorization rules requiring evidence of no unacceptable toxicity or disease progression; (5) Update provider education materials and encounter templates to clarify that all other indications beyond those listed are experimental/investigational and not covered; (6) Train authorization staff on distinction between FDA-approved vs. compendial uses. Failure to obtain authorization will result in claim denials. Note: Policy is approved for implementation on 9/30/26 but not before.
