Summary

Starting August 1, 2026, new providers joining the network must submit a W-9 form with both legal name and DBA name (if applicable), plus official IRS documentation (SS-4, 147C forms, or other IRS Tax ID notifications) as part of their onboarding application. Current in-network providers are not affected unless they have organizational changes.

By August 1, 2026: New provider onboarding staff must update application requirements to collect W-9 forms with legal name and DBA name plus official IRS Tax ID documentation (SS-4, 147C, or other IRS notifications). Current providers only need to resubmit if organizational changes occur (name or TIN changes). For demographic changes, out-of-network providers should use the Demographic Change Form.
