Summary
Medicaid medical transportation services will transition from Modivcare to MTM Health as the sole nonemergency medical transportation provider, effective November 1, 2026. Eligible members must contact MTM Health directly to request transportation services; existing recurring trips and scheduled trips after the effective date will automatically transfer to MTM Health.
Action Required
Before November 1, 2026: Billing and front desk staff must update patient education materials and referral processes to direct Medicaid members to MTM Health for nonemergency medical transportation. Contact information to provide to patients: MTM Health phone 866-913-4342, website, or MTM Link mobile app. For scheduling assistance, patients can call Ride Assist at 866-418-9829. No billing system updates required. Ensure all patient-facing materials (appointment confirmations, pre-visit instructions, eligibility forms) reflect the new provider. Verify that eligible members receive transition letters from the payer with MTM Health contact details.
