Summary

Excellus BlueCross BlueShield has designated a comprehensive list of intradiscal procedures as investigational and non-covered, effective October 15, 2025. This includes thermal techniques (IDET, nucleoplasty, radiofrequency annuloplasty), intradiscal injections (PRP, stem cells, ozone, etc.), and decompression procedures. Billing teams must immediately deny or request prior authorization denial for all claims involving these procedures and update internal systems to reflect non-coverage status.

Action Required

Action needed
By October 15, 2025: (1) Billing team must update revenue cycle management system to flag ALL claims for intradiscal procedures listed in policy 7.01.112 (annuloplasty, nucleoplasty, IDET/IDTA, PIRFT, biacuplasty, coblation, PDD, TDD, radiofrequency annuloplasty, targeted disc decompression, and intradiscal injections including methylene blue, hyaluronate, ozone, PRP, stem cells, glucocorticoids, growth factors, etc.) for denial based on investigational status. (2) Update encounter forms and provider templates to remove these as billable options for Excellus BlueCross BlueShield plans. (3) Notify all providers (especially spine surgeons, orthopedic surgeons, pain management specialists) that these procedures are non-covered and will be denied. (4) Configure billing software to automatically deny claims without requiring manual review. (5) Update patient financial counseling materials and pre-visit forms to inform Excellus patients that these procedures are not covered. (6) Train front desk and scheduling staff to inform patients during pre-authorization calls that intradiscal procedures are investigational and not covered. Failure to implement will result in claim denials, patient balance bills, and provider appeals burden.
