Action Required
At least 30 days before any ownership change: Providers must submit the Change Of Ownership (CHOW) Form to Highmark. Billing/administrative team must track receipt of Consent to Assignment (CA) or Contingent Consent to Assignment (CCA) documents and ensure both old and new owners sign within the strict 45-day timeframe (begins day form is received). Do NOT accept new Highmark members during the 45-day contractual process to prevent claims processing issues. Failure to meet these deadlines will result in the old provider profile being terminated and marked Out of Network, requiring the new operator to submit a Special Consideration Questionnaire with rare approval odds given Highmark's currently closed post-acute networks.
