Summary
Effective November 1, 2026, CPT code 93146 (carotid sinus baroreflex activation therapy device interrogation) now requires precertification/prior authorization through Carelon Medical Benefits Management for Anthem Blue Cross and Blue Shield commercial plans in Georgia. Providers must submit authorization requests via the Carelon provider portal, Availity Essentials, or email before performing this procedure.
Action Required
By October 1, 2026: Billing team must update systems to require prior authorization for CPT 93146 when billed to Anthem Blue Cross and Blue Shield commercial plans in Georgia. Cardiology providers and staff must be notified that this procedure requires precertification before scheduling or performing. Update encounter forms and billing software to flag CPT 93146 for prior auth submission. Prior authorization requests should be submitted through Carelon's provider portal (https://providerportal.com) or via Availity Essentials (https://Availity.com) for fastest processing.
