Summary
Anthem Blue Cross and Blue Shield (Ohio) is promoting the use of Alerts Hub, a new application within the Anthem Payer Space in Availity Essentials, to streamline care coordination for Medicare Advantage Special Needs Plans (SNPs) patients. This tool provides daily email notifications for admissions, discharges, transfers, and behavioral health events to reduce care coordination time and improve HEDIS and Stars metrics performance.
Action Required
By July 6, 2026 (or immediately upon implementation): Billing and clinical teams must access and configure Alerts Hub within Anthem Payer Space in Availity Essentials for all Medicare Advantage SNP patients. Providers and billing staff should complete the Digital Learning Hub ADT (Admission, Discharge, and Transfer Notifications) lesson to understand how to use daily alert notifications for care coordination. Update internal workflows to incorporate daily Alerts Hub messages into care coordination and Individual Care Plan (ICP) management processes. This is not mandatory but is strongly encouraged to optimize HEDIS and Medicare Stars performance metrics, which directly impact quality incentive programs and value-based care reimbursement.
