Summary
Molina Healthcare of California has updated all Community Supports referral forms to align with current DHCS requirements and established a new dedicated fax line (833) 305-3130 for CS referrals. Effective July 15, 2026, only the revised forms will be accepted; referrals submitted on outdated forms will be rejected and providers may be required to resubmit using current versions.
Action Required
By July 15, 2026: (1) Billing and referral staff must download and begin using ONLY the updated Community Supports referral forms from molinahealthcare.com/providers/ca/medicaid/forms/fuf.aspx. (2) Update all fax transmission procedures to route Community Supports referrals to the new dedicated fax line (833) 305-3130 instead of any previous fax numbers. (3) Remove outdated CS referral forms from all locations, EMR templates, and shared drives to prevent accidental use. (4) Train all staff responsible for submitting Community Supports referrals on the new form requirements and updated data elements. Consequence: Referrals submitted on outdated forms will be returned without processing, delaying member care authorization and creating rework. Processing timeframes will not begin until a complete referral is received on the updated form.
