Summary
Blue Cross of Idaho has consolidated provider policies and issued updated guidance on HCPCS add-on code G2211 (E/M complexity add-on). Critical change: G2211 is NOT reimbursed for Commercial, Connected Care Organization, or Qualified Health Plan lines of business—reimbursement applies to Medicare Advantage only. Effective January 1, 2025, G2211 becomes payable with modifier 25 in specific MA cases per CMS CR 13705.
Action Required
IMMEDIATE: Billing team must update claim submission rules to DENY or reject G2211 claims for all Commercial, Connected Care Organization, and Qualified Health Plan lines of business. By January 1, 2025: Update billing software to allow G2211 with modifier 25 ONLY for Medicare Advantage plans per CMS CR 13705 Attachment 1 (preventive services list). Ensure all claims for G2211 on MA products include: (1) valid E/M base code (CPT 99202-99205 or 99211-99215), (2) documentation of longitudinal care or ongoing management of serious/complex condition, and (3) medical necessity support. Providers and billing staff must review claim denials for G2211 on non-MA lines of business and resubmit without the add-on code or correct to appropriate MA product. Failure to implement will result in widespread claim denials and payment delays on non-MA products.
