Summary

Effective October 1, 2026, Harvard Pilgrim Health Care Commercial and Tufts Health Direct are reclassifying many colonoscopy-related medical conditions from preventive to diagnostic or therapeutic services. This means affected services will now require patient cost-sharing. When a preventive screening colonoscopy becomes diagnostic/therapeutic (e.g., biopsy, polypectomy), providers must bill with a preventive diagnosis code and modifier 33 in the first modifier field.

By September 15, 2026: Billing team must obtain and review the new Colonoscopy Medical Necessity Guidelines (MNG) from Harvard Pilgrim Health Care and Tufts Health Direct to identify which ICD-10 diagnosis codes trigger diagnostic vs. therapeutic vs. preventive classification. Update billing software logic to apply appropriate cost-sharing based on diagnosis code submitted with CPT codes 45385, 45380, and 45378. Train billing staff and providers that when a preventive screening colonoscopy converts to diagnostic/therapeutic during the procedure, modifier 33 MUST be appended to the first modifier field on claims along with a preventive diagnosis code. By October 1, 2026: Implement updated claim submission workflows to ensure compliant coding. Notify in-network providers of reclassification changes and modifier 33 requirement to prevent claim denials and patient billing disputes. Failure to follow these requirements will result in claim denials and potential patient balance disputes.
