REQUIREMENTS:
1. By June 1, 2026: Billing team must update prior authorization software to FLAG all claims for Itvisma (HCPCS J3405) for mandatory prior authorization review before submission. This affects Harvard Pilgrim Health Care Commercial, Tufts Health Plan Commercial, Tufts Health Direct, and Tufts Health RI Together members.
2. By August 15, 2026: Prior to the September 1, 2026 effective date, billing team and providers must review and implement the updated prior authorization criteria for respiratory interleukin drugs (Cinqair, Fasenra, Nucala, Tezspire, Xolair). Update encounter templates and billing system rules to enforce:
- Chart-confirmed diagnosis documentation requirements
- Disease severity assessment documentation
- Explicit concurrent therapy documentation
- Clear distinction between treatment failure vs. add-on therapy scenarios
- Minimum treatment duration standards for non-biologic therapies
- Standardized approval duration rules
3. Immediately: Providers must document all required elements (diagnosis confirmation, disease severity, concurrent therapy, treatment history) in the medical record for any respiratory interleukin authorizations to avoid claim denials.
4. Before July 1, 2026: Training must be provided to clinical and billing staff on new Itvisma authorization requirements and location of Medical Necessity Guidelines on Point32Health's provider website.
5. Effective July 1, 2026: Stop billing Beqvez and Roctavian; these drugs have been removed from the market and MassHealth's Acute Hospital Carve-Out Drug List.
6. By July 1, 2026: Update billing system for Elevidys to reflect expanded MassHealth coverage for patients over six years old on Tufts Health Together plans; verify new coverage criteria are correctly configured.
CONSEQUENCES: Claims submitted without required prior authorization or missing mandatory documentation will be denied. Failure to implement these changes will result in delayed payments, claim rejections, and potential compliance issues with Point32Health plans.
