Summary

The Indiana Health Coverage Programs (IHCP) is discontinuing the JG modifier for 340B drugs on medical claims effective August 1, 2026. Providers must use the TB modifier instead to identify 340B-acquired drugs on physician-administered drug claims for IHCP members. Claims submitted with the JG modifier for DOS on or after August 1, 2026 will no longer be accepted.

By July 31, 2026: Billing team must update all billing software, claim submission systems, and templates to replace the JG modifier with the TB modifier for all 340B-acquired drug claims submitted to IHCP members. Audit all standing orders, charge masters, and billing workflows that reference the JG modifier for 340B drugs. Train providers and billing staff on the modifier change. Implement system edits to reject or flag any claims attempting to use JG modifier for DOS on or after August 1, 2026. After August 1, 2026, all physician-administered drug claims identifying 340B-acquired drugs for IHCP must use TB modifier only, or claims will be denied.
