Summary

Independence Blue Cross updated its bioengineered skin substitutes policy effective 10/01/2026, revising the list of covered products for lower extremity ulcers, burns, and epidermolysis bullosa. Eight HCPCS codes (A2046, A2047, A2048, A2049, A2050, Q4207, Q4223, Q4243) moved from medically necessary to experimental/investigational status, while six codes (A2040, A2041, A2042, A2043, A2045, Q4298, Q4299) were revised on the policy attachment. Four G-codes (G0681-G0684) had their effective dates moved to 01/01/2026. Medical necessity criteria for chronic lower extremity wounds were clarified, and experimental/investigational uses (acute wounds, post-Mohs, post-surgical) were explicitly defined.

Action Required

Action needed
By 09/15/2026 (15 days before effective date): Billing team must immediately update billing system, fee schedules, and claim submission software to reflect coverage status changes. CRITICAL ACTIONS: (1) Flag and deny all claims for HCPCS codes A2046, A2047, A2048, A2049, A2050, Q4207, Q4223, Q4243 submitted after 10/01/2026 as experimental/investigational unless documented as meeting approved indications. (2) Update system edits for revised codes A2040, A2041, A2042, A2043, A2045, Q4298, Q4299 per Attachment A specifications. (3) Change effective date to 01/01/2026 for G-codes G0681, G0682, G0683, G0684 in billing system and apply retroactively to any claims submitted between 01/01/2026-10/01/2026. (4) Providers must document all five criteria for chronic lower extremity wounds including wound size ≥1.0 cm², clean granular base without tendon/muscle/bone involvement, absence of infection, adequate circulation (ABI ≥0.60 or toe pressure >30 mm Hg), and failed response to conservative care (4+ weeks for general wounds or diabetic ulcers with neuropathy; 3+ months for venous ulcers unresponsive for 30+ days). (5) Update encounter forms and EMR templates to capture required documentation. (6) Train billing staff that claims lacking this documentation will be denied. (7) Communicate to providers that acute wounds, post-Mohs procedures, and post-surgical uses are now explicitly experimental/investigational and will not be covered. Failure to implement these changes will result in claim denials, revenue loss, and potential compliance violations.
