Summary

Effective 12/1/2025, Providence Health Plan now requires specific clinical documentation for all electrolysis claims related to gender affirmation services on Commercial and Medicare lines of business. Documentation must include diagnosis of gender incongruence/dysphoria, WPATH SOC-8 alignment statement, clinical rationale, detailed treatment plan with session counts, and treatment area descriptions. This requirement does NOT apply to Providence OHP members.

By 11/15/2025 (before 12/1/2025 effective date): (1) Billing team must update billing system templates and claim submission processes to flag electrolysis claims for gender affirmation requiring documentation verification before submission. (2) Providers must be notified immediately of new documentation requirements through staff alerts and EMR/EHR updates. (3) Create and distribute a checklist to providers documenting all five required elements (A-E): diagnosis, WPATH SOC-8 statement, clinical rationale, treatment plan with area(s) and session counts, and treatment area description. (4) Front-line staff must screen all member reimbursement requests for electrolysis to ensure documentation is attached before processing. (5) Update denial response templates to cite missing required documentation if claims are submitted without complete information. Failure to include required documentation will result in claim denials. Note: Providence OHP members are exempt from these requirements—do not apply this policy to OHP claims.
