Summary

Effective March 1, 2026, Providence Health Plan will conduct targeted audits of practitioners billing high-level E&M services at rates significantly above peer averages. Providers with disproportionate high-level E&M billing compared to similar specialties may receive education outreach, and any high-level E&M claims lacking supporting clinical documentation will be subject to claim recoupment.

By February 28, 2026: Billing team must conduct internal audit of high-level E&M billing patterns (99214, 99215, 99233-99235, etc.) to identify outliers versus specialty peers. Providers must review and enhance clinical documentation standards to support medical necessity for all high-level E&M services, particularly in encounter notes, history of present illness, medical decision-making complexity, and time spent. Update EMR templates to include documentation checkboxes for high-level E&M encounters. Establish internal compliance monitoring to prevent unsupported high-level E&M billing. Be prepared for potential Providence Health Plan audit requests beginning March 1, 2026. Non-compliant claims may result in recoupment of previously paid amounts.
