Summary
Alabama Medicaid is conducting off-cycle provider revalidations effective 6/30/2026, with two tracks: swift revalidation for high-risk providers (DME, Home Health, Hospice, Nursing Facilities, ABA, Behavioral Health) and 24-month revalidation for moderate/limited-risk providers (33 provider types including hospitals, physicians, pharmacies, clinics). Providers will receive phased revalidation notices and must respond timely via the Medicaid Secure Web Portal or face enrollment termination.
Action Required
By 6/30/2026: All Alabama Medicaid enrolled providers must monitor for revalidation notices issued in phases and respond within the timeframe specified in their individual notice. Billing team should: (1) Flag all Alabama Medicaid provider agreements for tracking; (2) Designate staff to monitor the Medicaid Secure Web Portal (https://www.medicaid.alabamaservices.org/alportal/Account/Secure%20Site/tabId/22/Default.aspx) for revalidation notices; (3) Upon receipt of notice, immediately download the Revalidation Facsimile and required forms (Provider Agreement, Disclosure Forms, W-9, CLIA/DEA certificates if applicable, EPSDT/Plan First/Mammography agreements if applicable); (4) Complete all forms with current information; (5) Submit via Secure Web Portal before the deadline specified in the notice. High-risk providers (DME/Medical Supply, Home Health, Hospice, Nursing Facilities, ABA, Behavioral Health) expect expedited revalidation timelines. Failure to complete revalidation timely will result in loss of Medicaid enrollment and inability to bill. Contact Provider Enrollment at (888) 223-3630 for assistance.
