REQUIREMENTS: - Ongoing (90-day recurring cycle): All providers and facility administrators must log into https://Availity.com, navigate to My Providers > Provider Data Management, and attest to demographic data every 90 days by selecting 'Verify Directory Listing' and submitting the verified profile. - Designate at least one administrator responsible for tracking attestation deadlines. Administrators will receive automated email reminders and notifications in the Availity Essentials Notification Center when attestation is due or overdue. - If not yet registered for Availity Essentials: Complete registration immediately at https://Availity.com (no cost). Ensure all TINs associated with the organization are registered. - Behavioral health providers assigned to Carelon Behavioral Health, Inc. must follow Carelon's attestation process instead. CAQH-participating providers must attest through the CAQH website. - CONSEQUENCES: Failure to complete 90-day attestation will result in non-compliance classification and removal from the online provider directory, preventing members from finding and accessing the practice through Anthem's Find Care tool.
