Summary
HHSC has revised Form 2032 for Escheatment of Consumer Funds effective June 2026, with an updated mailing address for HHSC Accounts Receivable. Assisted living facilities, intermediate care facilities for people with intellectual or developmental disabilities, and nursing facilities must immediately discard older form versions and use only the revised Form 2032 (identified by version date on top right). Update any automated systems, templates, or processes that generate or store this form to use only the revised version. Confirm the new HHSC Accounts Receivable mailing address is correctly entered in billing software and correspondence systems. Failure to use the current form version will result in significant delays in processing escheatment submissions.
