Billing chart: Blue Cross highlights medical, benefit policy changes

By July 1, 2026: Billing team must access and review the full July 2026 Billing Chart from Blue Cross Blue Shield of Michigan. (1) Log into availity.com and navigate to Benefit Explainer under the Commercial Policy tab to review procedure-specific policy changes organized by HCPCS Code. (2) Visit bcbsm.com/providers > Resources > Search medical policies to review updated medical policies for both commercial and Medicare Advantage plans. (3) Identify all affected procedure codes under these categories: New Payable Procedures, Updates to Payable Procedures, Policy Clarifications, and Experimental Procedures. (4) Update billing software, encounter forms, and internal guidelines to reflect all code changes and new billing requirements. (5) For Medicare Advantage plans, cross-reference the Medical Policy Router to identify separate MA policies that may override or supplement commercial policies per CMS compliance (42 CFR § 422.101[c]). (6) Train providers and billing staff on any changes to payable procedures, guidelines, and non-covered experimental procedures. Failure to implement changes timely will result in claim denials and payment delays.
