Update internal billing guidelines and fee schedules to reflect the integrated policies. For Outpatient Facility claims: verify understanding of new revenue code grid and SG modifier usage requirements—update claim submission procedures accordingly. Notify all providers and billing staff of policy consolidation and effective date. Failure to implement these changes will result in claim denials or payment delays after September 1, 2025, as legacy policies will no longer be valid.
