Summary
Tufts Health Plan Senior Care Options (SCO) PCPs and specialists must complete the updated 2025 SCO Model of Care training and submit an attestation by December 31, 2025. Action Required
By December 31, 2025: All PCPs and specialists participating in Tufts Health Plan Senior Care Options (SCO) plans must complete the new 2025 SCO Model of Care training available in the Training Section of the Point32Health provider website. Upon completion, providers must submit the attestation verification. Billing team should: (1) Create a tracking log to monitor which providers have completed training by the deadline, (2) Flag any claims from non-compliant providers for review, as CMS requires this training for plan participation, (3) Send reminders to all SCO providers by October 1, 2025 to allow adequate completion time. Non-compliance may result in provider termination from the SCO plan and subsequent claim denials.
