Summary

Effective July 1, 2026, Providence Health Plan is transitioning behavioral health services from Carelon Behavioral Health to in-house management. Providers contracted directly with Providence or participating in Cigna's PPO/First Choice networks will remain in-network. Out-of-network providers will receive transition-of-care support letters, and members will receive new ID cards with updated claims submission addresses by July 1, 2026.

Action Required

Before Jul 1, 2026
By June 30, 2026: (1) Billing team must verify your behavioral health provider contract status with Providence Health Plan—contact 800-878-4445 to confirm if your practice is contracted directly with Providence or participates in Cigna PPO/First Choice networks to maintain in-network status. (2) Update claims submission addresses in billing software to reflect new Providence Health Plan behavioral health claims addresses based on plan type and network affiliation (new ID cards with addresses will be distributed by July 1). (3) Establish process to monitor incoming transition-of-care support letters for out-of-network providers and implement continuity-of-care protocols. (4) Train front desk and billing staff on new claims submission procedures and updated ID card verification. Failure to submit claims to correct addresses after July 1 may result in claim denials or delays. Contact Providence Provider Relations at 800-878-4445 (Monday-Friday, 8 a.m.–5 p.m. Pacific Time) with questions about network participation or billing address changes.
