REQUIREMENTS: 1. By June 30, 2026 (DEADLINE): All physical health providers must verify directory information in the Presbyterian provider portal. Behavioral health providers must verify in the behavioral health portal. This is a federal No Surprises Act requirement with no exemptions. Contact providerdemo@phs.org (physical health) or PHPTCBH@magellanhealth.com (behavioral health) for assistance. 2. Immediate: Billing and clinical teams should implement processes to ensure annual eGFR and uACR testing documentation for all diabetic patients to support the KED HEDIS measure. Update encounter templates and EMR workflows to prompt providers to order and document both tests. Educate providers that uACR requires quantitative urine albumin tests (not dipsticks) within four or fewer days of creatinine testing. 3. Immediate: Audit medical records for advance directive documentation deficiency (currently 27.2% compliant). Implement requirement that all patients 18+ have documentation showing: (a) patient education about advance directives, (b) patient declination, or (c) signed advance directive receipt with date and method. Update intake forms and EMR templates accordingly. 4. Immediate: Ensure immunization records include documented dates for all administered vaccines or notation that member is up-to-date or has declined further immunizations. Review chart audits for immunization documentation gaps, particularly for network providers (currently 82.1% compliant). 5. Before June 30, 2026: All contracted providers must register for and attend at least one Provider Education Conference & Webinar Series training (physical health: June 10-11; behavioral health: June 10-11). Billing team should track compliance and flag providers for follow-up if not completed.
