Summary

Healthy Alliance is launching a Social Care Network (SCN) across the Capital Region, Central New York, and North Country to address social determinants of health for Medicaid members. Providers can become contracted partners to screen, navigate, and deliver services (food, housing, transportation, utilities) and receive reimbursement for eligible Medicaid members. The SCN aims to achieve 100% screening of attributed Medicaid population.

By November 30, 2025: Determine if your practice serves attributed Medicaid members in Capital Region, Central New York, or North Country areas. If interested in contracting as a screener, navigator, or service provider, submit application through Healthy Alliance website to enable billing/reimbursement for SCN services. Billing team must establish new revenue cycle processes for social care service reimbursement claims once contracted. Contact your MVP Provider Relations Representative or Provider Partnership Liaison for eligibility details and to request application access. Failure to contract will prevent reimbursement for any SCN screening or navigation services provided to eligible Medicaid members.
