Summary
BlueSpeak June 2026 introduces two new medical policies effective 7/1/2026: (1) Radiology Site of Care—advanced diagnostic imaging at hospital-based outpatient facilities may be denied if the service can be safely performed at a freestanding facility and site-of-care criteria are met; (2) Fractional CO2 Laser Ablation for Scars—treatment is medically necessary only when documented functional impairment exists, at least two other scar treatments have been trialed, and specific clinical criteria are met. Billing teams must implement prior authorization workflows and documentation requirements for both policies.
Action Required
By June 30, 2026: (1) Billing team must update authorization system to flag advanced diagnostic imaging claims performed at hospital-based outpatient facilities for site-of-care review; coordinate with radiology providers to ensure imaging services are scheduled at freestanding facilities when clinically appropriate and site-of-care criteria are met. (2) Billing team must configure claim submission to require documentation of: significant functional impairment related to scar, evidence of functional improvement expected from treatment, and documentation that at least two prior scar treatments (intralesional corticosteroids, pressure garments, silicone gel/sheeting, or other) have been trialed. Update encounter forms and clinical templates to capture these required elements. Claims lacking required documentation will be denied. Front desk and providers must be notified of new documentation requirements. Verify these changes apply to all affected lines of business (Commercial, ACA QHP, Small Group, ACA JAA, FEP Medicare Advantage).
