Anthem Blue Cross and Blue Shield Ohio Medicaid updated clinical criteria for 26 specialized medications effective October 1, 2026. Review each for changes to medical necessity requirements and document submission standards. Identify which medications are commonly prescribed in your practice. Coordinate with providers and prior authorization teams to understand new/revised requirements. Update internal guidelines, encounter forms, and billing software authorization rules as needed. Watch for separate authorization requirements notice from Anthem. Failure to comply with revised clinical criteria may result in claim denials for Ohio Medicaid members.
