Summary

Effective immediately (June 24, 2026): Billing team must audit and restructure all Medicare Advantage claim submission processes to ensure no single claim exceeds 449 service lines per encounter. Implement one or more of the following strategies in the billing system: (1) Combine service lines where clinically appropriate, (2) Split outpatient claims by date of service, or (3) Submit inpatient claims as interim bills. Update claim validation rules in billing software to flag and prevent submission of claims with >449 service lines. Train billing staff on these limits and remediation strategies. Providers must review documentation practices to support line combining where possible. Failure to comply will result in claim rejections from Anthem Blue Cross and Blue Shield Medicare Advantage plans.
