Summary

Humana Medicare Advantage has updated its Solosec (secnidazole) prior authorization policy effective April 22, 2026. The policy maintains strict prior authorization requirements for bacterial vaginosis and trichomoniasis treatment, requiring documented failure, contraindication, or intolerance to first-line agents (metronidazole and clindamycin for BV; metronidazole and tinidazole for trichomoniasis) before Solosec can be approved. This is a revision of the existing policy rather than a new coverage change.

Action Required

Action needed
By April 22, 2026: Verify that billing and prior authorization systems are configured to enforce Solosec prior authorization requirements for Medicare Advantage members. Ensure that: (1) Claims for Solosec require prior authorization submission; (2) Prior auth requests must include documentation of patient's previous treatment with, contraindication to, or intolerance of metronidazole AND clindamycin (for bacterial vaginosis) OR metronidazole AND tinidazole (for trichomoniasis); (3) Providers document the specific reason (treatment failure, contraindication, or intolerance) for the alternative agent in the medical record. Update provider documentation templates and billing software rules if needed. Notify providers of the specific prior auth requirements. Claims submitted without proper prior authorization or missing required documentation will be denied.
