Summary

Immediately: Billing team must implement workflow changes for Humana Medicare Advantage claims. (1) Monitor pharmacy claims for automatic rejections related to lifetime dosing limits on select drugs—these will trigger at point-of-sale. (2) When claims are stopped for clinical review, providers must submit written attestation of medical necessity along with clinical rationale and compendia support through Humana's PAL system (www.humana.com/PAL). (3) Establish internal process to track which drugs have lifetime limits in Humana's system and flag prescriptions approaching or exceeding limits before submission. (4) Update billing software to capture and forward provider clinical justification with claims exceeding lifetime limits. (5) Set expectations with providers that exceptions are approved only through end of plan year and require documented proof that member failed to reach treatment goals with standard dosing. Failure to provide required clinical documentation will result in claim denials.
