Summary
Humana has revised its Soaanz® (torsemide) pharmacy coverage policy effective January 28, 2026. This is a step therapy policy for Medicare Advantage members requiring prior authorization. Members must have prior treatment with at least two of the following loop diuretics before Soaanz approval: furosemide, bumetanide, or torsemide. The policy applies to treatment of edema associated with heart failure or renal disease.
Action Required
By January 28, 2026: Billing and pharmacy teams must update prior authorization workflows to enforce Soaanz® (torsemide) step therapy requirements for all Medicare Advantage members. Before processing claims for Soaanz 40 mg tablets, verify in member records that the patient has received prior treatment with at least two of the following: furosemide tablet, bumetanide tablet, or generic torsemide tablet. Update authorization request templates and pharmacy systems to document which two loop diuretics the member previously used. Train billing staff and prior auth specialists on the new requirement. Communicate the requirement to participating providers and pharmacies. Failure to obtain proper prior authorization based on step therapy criteria will result in claim denials. This policy supersedes any previous Soaanz coverage guidance.
