Summary

This is a revised Medicare Advantage prior authorization policy for topical tazarotene products (Arazlo lotion and Fabior foam) used to treat acne vulgaris. The policy requires members to have a documented diagnosis of acne vulgaris AND prior treatment with, intolerance, or contraindication to tretinoin (non-micro) or adapalene 0.1% gel before coverage is approved. Cosmetic uses are excluded.

Immediately: Billing and prior authorization teams must implement this policy for all Humana Medicare Advantage members requesting Arazlo (tazarotene lotion) or Fabior (tazarotene foam). Before submitting claims or authorizations: (1) Verify the member has a documented diagnosis of acne vulgaris; (2) Confirm prior treatment with tretinoin (non-micro) or adapalene 0.1%, OR document intolerance/contraindication to these agents; (3) Deny any claims for cosmetic indications (solar lentigines, wrinkles, skin roughness without approvable acne diagnosis); (4) Route all requests through prior authorization process with plan year duration approval. Update encounter forms and billing workflows to include these verification requirements. Claims submitted without meeting these criteria will be denied. Note: This policy was revised on February 25, 2026—verify with Humana that this is the current version before processing claims.
