Summary

Humana Medicare Advantage updated its Dopamine Agonists step therapy policy (revised 11/26/2025). The policy requires members to have tried or be unable to use immediate-release pramipexole AND immediate-release ropinirole before approval for extended-release formulations (Mirapex ER, pramipexole ER, ropinirole ER) or Neupro patches. This is a step therapy requirement that mandates prior authorization for these medications.

Immediately: Prior to submitting pharmacy claims for Mirapex ER, pramipexole ER, ropinirole ER, or Neupro (rotigotine) patches to Humana Medicare Advantage plans, the billing/pharmacy team must verify that the member has documentation of prior trial or contraindication to BOTH immediate-release pramipexole tablets AND immediate-release ropinirole tablets. Update pharmacy prior authorization workflows and claim submission procedures to require this step therapy documentation. Without evidence of meeting Criterion #1 (prior trial or inability to use both immediate-release agents), claims will be denied. Coordinate with prescribing providers to ensure they document treatment history with immediate-release dopamine agonists on all Mirapex ER, pramipexole ER, ropinirole ER, and Neupro requests submitted to Humana MA plans.
