Summary
Independence Administrators updated the disclaimer language in policy 06.02.36d to clarify that molecular testing (PathFinderTG®, PancraGEN®, BarreGEN®) for pancreatic cysts and Barrett's esophagus is NOT covered for members whose groups have enrolled in the UM vendor program. The policy maintains its experimental/investigational status and non-coverage position. This is a reissue with no substantive clinical changes, only administrative clarification of applicability.

REQUIREMENTS:
- By June 24, 2026: Billing team must update claim submission and authorization protocols to confirm whether each patient's group has enrolled in Independence Administrators' UM vendor program before processing claims using CPT 84999 or 89240 for molecular testing (PathFinderTG®, PancraGEN®, BarreGEN®).
- For groups NOT enrolled in UM vendor program: Continue applying the experimental/investigational exclusion (deny claims).
- For groups enrolled in UM vendor program: Do NOT apply this policy; refer to alternative coverage guidance or eviCore Lab Management Program instead.
- Billing team must verify individual member benefits in the benefit contract BEFORE providing services to determine group enrollment status in UM vendor program.
- Update internal policy documentation and billing system notes to reflect the new disclaimer language that distinguishes between UM vendor enrolled and non-enrolled groups.
- Failure to verify enrollment status may result in claims being processed under incorrect policy parameters or member appeals.
