Summary
Humana Medicare Advantage now requires step therapy for specialty calcium channel blocker formulations (Norliqva, Katerzia, Sdamlo). Members must first try two generic calcium channel blockers (amlodipine tablet, verapamil tablet, or diltiazem tablet) before these liquid/powder formulations will be covered.

Immediately: For Humana Medicare Advantage patients prescribed Norliqva, Katerzia, or Sdamlo, verify the patient has previous treatment with at least two generic calcium channel blockers (amlodipine tablet, immediate release verapamil tablet, or immediate release diltiazem tablet) before submitting pharmacy claims. Document this trial history in patient records. Claims without documented step therapy compliance may be denied.
