Summary
Humana has updated their prior authorization policy for Breztri Aerosphere inhaler for COPD treatment. The policy now requires patients to have tried and failed Trelegy Ellipta plus one other combination inhaler before Breztri will be approved.
Action Required
Immediately: When prescribing Breztri Aerosphere for COPD patients, ensure documentation shows patient has tried and failed or has contraindications to both Trelegy Ellipta AND one ICS/LABA combination (Breo Ellipta, Advair HFA) OR one LAMA/LABA combination (Anoro Ellipta, Stiolto Respimat). Update prior authorization request forms to include this step therapy documentation. Claims will be denied without proper prior authorization.
