Action Required
By August 1, 2013: All NEMT providers (provider type 28) must submit completed standard Daily Trip Reports with every Fee for Service claim. Billing team must ensure reports are 8.5 x 11, black and white, include full addresses, times, and odometer readings for each transport segment. Claims submitted without proper trip reports will be denied and must be corrected and resubmitted.
