Ensure prescribing dermatologist or rheumatologist documents: 1) diagnosis of generalized pustular psoriasis currently NOT in flare, 2) prior therapy failure/intolerance/contraindication to oral systemic treatments (acitretin, methotrexate). Failure to obtain prior auth will result in claim denials.
