Summary
AHCCCS (Arizona Medicaid) has implemented several billing changes including UB-04 date validation starting July 1, 2014, delayed ICD-10 transition until October 1, 2015, and new ADA 2012 dental form requirements by August 1, 2014. Claims with date discrepancies will be denied and ICD-10 codes will be rejected until the official transition date.
Action Required
By July 1, 2014: Billing team must ensure all UB-04 claims have line-level service dates (Field 45) within the header date range (Field 6) to avoid automatic denials. Continue using ICD-9 codes only until October 1, 2015 - do not submit ICD-10 codes as they will be rejected. By August 1, 2014: Dental providers must transition to ADA 2012 claim forms only. Stop billing epidural code 01996 on the same day as major procedures per CCI guidelines.
