Summary
AHCCCS (Arizona Medicaid) has implemented several billing code changes: HCPCS L8680 will be end-dated effective 3/1/15 as payment is now bundled into CPT 63650, and CPT 99188 will replace HCPCS D1206 for fluoride varnish application by physicians (non-dentists) effective 4/1/15. The policy also reminds providers not to resubmit pending claims and reinforces the ICD-10 transition date of 10/1/2015.
Action Required
By March 1, 2015: Stop billing HCPCS L8680 for disposable leads as payment is now bundled into CPT 63650. By April 1, 2015: Provider types 08, 18, 19, and 31 must switch from HCPCS D1206 to CPT 99188 for fluoride varnish application - dentists (type 07) continue using D1206. Update billing system rules to prevent duplicate billing of both codes. Until April 1st, either code may be used but not both for the same service.
