By May 28, 2025: Before prescribing Tezruly (terazosin oral solution) for Medicare Advantage patients, providers must obtain prior authorization by documenting diagnosis of BPH or hypertension and previous treatment failure or intolerance to at least one generic alpha-1 blocker (alfuzosin, doxazosin, prazosin, terazosin). For hypertension cases, also document trial of ACE inhibitor, ARB, calcium channel blocker, or diuretic. Update EMR templates to include these requirements.
