Summary
Humana Medicare Advantage now requires prior authorization for Tryptyr (acoltremon ophthalmic solution), a new dry eye medication. Patients must have tried and failed cyclosporine eye drops AND Eysuvis before approval for this new treatment option.
Action Required
Before September 24, 2025: Billing and clinical staff must update prior authorization workflows for Tryptyr (acoltremon ophthalmic solution). Providers must document diagnosis of dry eye disease and previous treatment failure or contraindication to both cyclosporine eye drops (generic Restasis) AND Eysuvis (loteprednol) before prescribing. Update EMR templates to include required documentation. Claims without prior auth will be denied.
