Humana Medicaid Louisiana has implemented new restrictions on foot bath medications (budesonide, ciclopirox, clindamycin, fluocinonide, ketoconazole, linezolid, nystatin, vancomycin) used as foot soaks. These medications will now require label or compendia support and will trigger automatic rejections when multiple foot bath medications are dispensed together.

Effective October 15, 2025: Billing team must ensure any foot bath medications (budesonide, ciclopirox, clindamycin phosphate, fluocinonide, ketoconazole, linezolid, nystatin, vancomycin) have label or compendia support before prescribing for Louisiana Medicaid members. Notify providers that multiple foot bath medications dispensed together will trigger automatic rejections with error code 88/527 requiring clinical pharmacy review.
