By August 15, 2016: All licensed Board Certified Behavior Analysts (BCBAs) must submit provider registration packets to AHCCCS Provider Registration. Contact Angelica Quezada at (602)417-4098 or fax to (602)256-1474. Starting July 26, 2016: Begin accepting KidsCare applications for coverage beginning September 1, 2016. Verify KidsCare eligibility and submit claims to enrolled health plans, not AHCCCS Fee-For-Service. Claims will be denied after October 1, 2016 without active AHCCCS-registered BCBA provider registration.
