Humana Medicare Advantage plans will require prior authorization for Uptravi (selexipag) starting January 1, 2026. Members must have confirmed pulmonary arterial hypertension and have tried or been unable to use an endothelin receptor antagonist first.

Before January 1, 2026: Billing team must update prior authorization processes for Uptravi (selexipag) prescriptions for Humana Medicare Advantage patients. Ensure providers document pulmonary arterial hypertension diagnosis confirmed by right heart catheterization and prior therapy/intolerance/contraindication to endothelin receptor antagonists. Submit prior authorization requests through www.humana.com/PAL before prescribing.
