By January 1, 2026: Billing team must implement prior authorization requirements for topical doxepin products (doxepin, Prudoxin, Zonalon) for Humana Medicare Advantage patients. Providers must document diagnosis of atopic dermatitis or lichen simplex chronicus AND previous treatment with, contraindication, or intolerance to at least two of the following: triamcinolone, mometasone, fluticasone cream, betamethasone, or clobetasol cream/ointment. Update EMR templates to capture required documentation. Claims will be denied without proper prior authorization.
