For DPN patients, document diabetic peripheral neuropathy of feet diagnosis and previous treatment with at least two of: gabapentin, duloxetine, lidocaine 5% patch, or pregabalin. Update EMR templates to capture required documentation. Claims without proper prior authorization will be denied.
