Summary
Humana Medicare Advantage now requires prior authorization for buprenorphine weekly transdermal patches for moderate to severe pain management. Patients must meet three specific criteria including persistent pain requiring around-the-clock treatment, inadequate response to alternative treatments, and previous treatment/contraindication to immediate or extended-release opioids.
Action Required
By January 1, 2026: Billing team must implement prior authorization requirements for buprenorphine weekly transdermal patches for Humana Medicare Advantage members. Providers must document: (1) persistent non-acute pain requiring around-the-clock treatment, (2) pain severity requiring opioid medication with inadequate response to non-opioid analgesics or immediate-release opioids, and (3) previous treatment, contraindication, or intolerance to immediate-release or extended-release opioids (oxycodone, hydromorphone, or morphine). Update EMR templates to capture required documentation. Claims without prior authorization will be denied.
