Humana Medicare Advantage implemented step therapy requirements for Auvelity (dextromethorphan/bupropion) for major depressive disorder and Alzheimer's agitation. Prior authorization requires documented trial of generic SSRI, SNRI, bupropion, or mirtazapine first, except for Alzheimer's agitation cases.
Action Required
By January 1, 2026: Providers must document prior therapy with generic SSRI (citalopram, fluoxetine, paroxetine, sertraline), SNRI (venlafaxine, duloxetine), generic bupropion, or mirtazapine before prescribing Auvelity for MDD patients. Update EMR templates to include depression medication history. Prior authorization will be denied without documented step therapy. Exception: No step therapy required for agitation associated with Alzheimer's dementia.
